So if I understand the manual right, we are all Captains of one grade of another. No First Officers.

Correct. All pilots when starting out will hold a grade of Captain with a B737 type rating. When the pilot has the flight hours for the next type aircraft rating they may begin to fly that aircraft (if they wish) as the "First Officer" to become familiarize with the new aircraft type. When the pilot feels confident in their abilities in this new type aircraft, they may take the check ride in that type aircraft. After passing the check ride the pilot will now hold that type rating and be able to "Captain" that aircraft. We will not be addressing anyone as "First Officers" during this transition period between type aircraft grades.
